Paper form design and supporting artwork
Professional graphic design. Specialist form expertise. Materials designed around your respondents.
We combine professional graphic design with specialist form layout to create paper surveys, questionnaires and forms that look polished, suit the people completing them and make efficient use of every page.
You provide the questions and other written content. We take care of the visual design, layout and artwork - bringing together your branding, your audience's needs and the practical requirements of printing and data capture.
Professional graphic design - with a purpose
Your survey materials should reflect the quality and professionalism of your organisation.
Our graphic designers bring together typography, colour, imagery and page composition to create an attractive, focused design. Whether the brief calls for a welcoming community questionnaire, a restrained research survey or a coordinated set of participant materials, we give the presentation the attention it deserves.
But appearance is only part of the brief. We combine that creative capability with specialist knowledge of paper forms, so the visual design supports the task: helping people find their way through the material, follow the instructions and record their answers.
Professional presentation and practical form design, considered together from the start.
Designed for the people filling it in
A questionnaire for older respondents may need a different approach from one used in a classroom or workplace. We plan the layout around the people completing the form and the circumstances in which they will use it.
That includes readable type, appropriate contrast, clear spacing and answer areas suited to the responses required. We pay attention to how questions, instructions and response options sit together, and how people move through the pages.
Where required, we can work to supplied accessibility and clear print guidelines, including requirements for older respondents and people with low vision. This may call for larger text, more generous answer spaces or a different approach to page layout.
The aim is a form that is clear and straightforward to use - not simply one that looks good or meets a scanner's specifications.
Better use of space. Often substantially fewer pages.
We often substantially reduce the page count of questionnaires supplied as Word documents or existing layouts - including versions previously prepared by other professional suppliers.
The improvement comes from specialist layout: more effective use of page space, consistent formatting, carefully planned response areas and well-managed page breaks. Your questions and response options are retained; the layout is what changes.
We look for wasted space - not opportunities to squeeze the reader.
A more compact layout can mean fewer printed sheets and a less bulky survey pack. But reducing pages is never the only objective. Readability, accessibility and adequate room for answers remain central to the design.
Sometimes the best result is a significantly shorter form. Sometimes it is a clearer, more usable form with the same number of pages.
Your brand, style and print requirements
We can work to your organisation's brand guidelines, visual identity or style guide, print specifications and relevant accessibility or clear print guidelines.
That means much more than adding a logo. We can apply your approved fonts, colour palette, imagery, layout conventions, logo placement and co-branding requirements across the questionnaire and accompanying materials.
You may have a comprehensive corporate style guide, a project-specific visual identity or simply a set of existing materials the new work needs to match. We use what you provide to establish a consistent design approach.
Where branding, readability and scanning requirements need to be reconciled, we discuss the options with you before the artwork is finalised.
Professional design for the complete survey pack
Our design service extends beyond the questionnaire itself.
Using your supplied content, we can prepare professional graphic design and print-ready artwork for supporting materials, including:
- Letters: covering letters, survey invitations, pre-approach letters and reminders.
- Information materials: participant information sheets, instruction sheets and handouts.
- Postcards: invitations, reminders and other project communications.
- Envelopes and inserts: outgoing envelopes, Reply Paid envelopes and accompanying printed materials.
These materials can be included in the project scope as required, whether you need a single supporting item or a coordinated survey pack.
The questionnaire, instructions, letters and envelopes can all follow the same visual approach, with each item designed for its particular purpose.
Built for printing and reliable data capture
For automated scanning, we design and print forms to our Scantron specifications. The artwork incorporates correctly positioned response areas, scanner-readable marks and page identifiers. Response areas use dropout colours - colours the scanner ignores - with respondent IDs, barcodes, QR codes and handwriting-capture areas included where required.
Formats include A4 sheets and folded or saddle-stitched booklets. Address panels and return arrangements are considered within the layout.
Existing questionnaires can be rebuilt for this process. We do not accept externally designed or printed forms for automated scanning; manual data entry may be an alternative for smaller runs.
From your content to approved artwork
Start with your content and requirements.
Send us your questions, draft document or existing questionnaire, together with the text for any supporting materials. Include your brand assets, style guide and any print or accessibility requirements. Tell us who will complete the form and how the materials will be used.
We bring the design together.
We develop the graphic design and specialist layout, considering presentation, usability, page efficiency and production requirements as one brief. Your supplied question wording and response options remain under your control; any queries are referred back to you.
You review the artwork before production.
We provide a PDF proof, incorporate agreed corrections and obtain your approval before anything is printed.
Have a draft questionnaire or an existing form?
Send us what you have, along with the approximate quantity, your timing and any supporting materials you need.
We'll discuss how the design and layout can work for your respondents, your branding and the way the project will be delivered.
